See All Operation LogsSinkhole
On 23 August 2024, at approximately 0800 hours, a sinkhole suddenly formed along a pedestrian walkway in front of Wisma Melayu, Kuala Lumpur. The collapse engulfed an Indian national, who disappeared from sight after falling into the cavity. Initial assessments indicated the sinkhole was approximately eight meters deep. Lt. (CD) Wei Xian and his team were immediately deployed to the scene with specialized equipment. Upon arrival, rescuers faced a highly complex and perplexing situation. Despite the depth of the sinkhole, there was no visible secondary opening or clear pathway within the cavity that could explain the victim’s disappearance. Search and rescue operations were conducted in close coordination with local authorities and the sewage management company. The team systematically inspected and opened multiple manholes along the drainage network and carried out targeted excavation in several locations in an effort to trace the victim’s path. Despite exhaustive technical efforts, no recovery was made. After all feasible rescue avenues had been thoroughly explored, the operation was formally concluded. The government subsequently declared the victim deceased and issued a death certificate to her family.

Fallen Tree
The Klang Valley is known for its extreme weather contrasts—scorching heat on clear days and intense storms capable of causing significant destruction. On 7 May 2024, following severe rainfall and strong winds, a massive tree was uprooted at approximately 1419 hours, crashing onto a busy roadway. The incident claimed one life, left another individual injured, and damaged 17 vehicles, including impact to the elevated monorail track above. Lt. (CD) Wei Xian and his team were immediately deployed to manage the scene and conduct hazard mitigation operations. Working alongside local authorities, the team undertook the complex task of removing the fallen tree and clearing debris. Due to structural concerns and obstruction caused by the fallen trunk, monorail services were temporarily suspended. The operation required high-risk work at elevation to safely remove debris from the rail alignment and restore safe operating conditions. After several hours of coordinated effort, the roadway and monorail track were successfully cleared, enabling the safe resumption of public transport and the restoration of normal city operations.
Kapar Plane Crash
On 13 February 2024, Lt. (CD) Wei Xian and his team were alerted to another aviation incident at Kampung Tok Muda, Kapar. The team was immediately deployed to the crash site, where the pilot and a passenger were found buried approximately two meters beneath the wreckage. Operating under technically demanding conditions, the team worked in close coordination with the Forensic Team from the Ministry of Health and the Royal Malaysia Police to conduct a meticulous search and recovery operation. Due to the depth of burial and compacted debris, the extraction required precision, controlled dismantling, and careful handling to ensure the intact recovery of both victims. Despite the complexity of the operation, the mission was carried out with professionalism, discipline, and respect for the deceased.

Blue Valley Landslide
On 26 January 2024, at approximately 0210 hours, a landslide struck Blue Valley, Cameron Highlands. The Civil Defense District Officer requested reinforcement from Civil Defense Headquarters for a Search and Rescue (SAR) team equipped with advanced tools to locate and recover five missing victims. An initial team was deployed in the morning. However, as the scale and logistical demands of the operation became clear, additional manpower and supplies were required. Lt. (CD) Wei Xian and his team were subsequently mobilized to the site, providing critical logistical support and operational reinforcement. The landslide area was located deep within the valley, requiring rescue personnel to trek approximately 3–5 kilometers, depending on the access route, before reaching the impact zone. The terrain was expansive, unstable, and technically challenging, significantly complicating search efforts. Through close cooperation with local villagers, the expertise of multi-agency rescuers, and the deployment of specialized equipment, all five deceased victims were successfully located, recovered, and brought out from the valley. The remains were then formally handed over to the police for further procedures.

Elmina Plane Crash
On 17 August 2023, at approximately 1500 hours, Lt. (CD) Wei Xian was informed of an aviation incident involving a flight, shortly after he had just transported a casualty from an earlier crash to the hospital. While still at the hospital, he received confirmation of the incident from the Civil Defense National Command Center. Without delay, Lt. (CD) Wei Xian immediately mobilized his team and proceeded to the disaster site. At around 1830 hours, following the completion of police evidence collection, the team entered the area and established operational lighting. Working in close coordination with the Forensic Team from the Ministry of Health and the Royal Malaysia Police, the team conducted a systematic recovery operation. Using advanced rescue equipment, they dismantled aircraft wreckage and successfully retrieved all human remains within approximately two and a half hours. The team then re-entered the site to recover additional body parts separated from the main remains, ensuring a thorough and dignified operation. This tragic incident resulted in the loss of 10 lives.
Türkiye Earthquake
On 6 February 2023, the deadliest earthquake and most catastrophic natural disaster in Türkiye’s modern history struck the nation, claiming more than 53,537 lives. Lt. (CD) Wei Xian deployed on 11 February 2023, joining the Subic Bay Metropolitan Authority Search and Rescue Team and United States CODE24 in a coordinated international search and rescue mission. Operating in extreme conditions with temperatures dropping to –14°C, the environment was harsh, unstable, and physically punishing. Despite the freezing climate and complex disaster terrain, Lt. (CD) Wei Xian and his team remained resolute and mission-focused. Through disciplined and persistent operations, they successfully recovered four victims from the rubble. For Lt. (CD) Wei Xian, this deployment carried profound personal meaning. His calling to serve in disaster response was first awakened during the 2004 earthquake and tsunami, when he was just 10 years old. After 19 years of preparation, training, and unwavering commitment, he answered that calling on the international stage—serving with honor in one of the world’s most challenging humanitarian crises.
Batang Kali Landslide
On 18 December 2022, at approximately 0300 hours, Lt. (CD) Wei Xian and his team were alerted by the Civil Defense District Officer to a major landslide disaster, with initial reports indicating that more than 100 victims were trapped. Without hesitation, the team immediately mobilized and deployed to the site, commencing an intensive 8-day, 7-night search and rescue operation. Working alongside rescue units from across the nation under extremely challenging and devastating conditions, the team remained calm and focused despite widespread panic and distress among victims. Through perseverance and coordinated efforts, all 31 deceased victims were successfully located and recovered, bringing the mission to a dignified and successful conclusion.

Oxley Tower Collapsed
On 27 September 2022, at approximately 0212 hours, Lt. (CD) Wei Xian and his team were deployed to Oxley Tower following reports of a structural collapse on the 51st floor of an under-construction building. A concrete structure had failed, trapping one worker beneath the debris. Upon arrival, the scene presented significant operational challenges. The collapsed concrete had flattened the surrounding area, leaving no visible indication of the victim’s location. The site was heavily cluttered and unstable, requiring systematic debris removal while awaiting confirmation from the K9 search unit. Once the canine provided a positive indication, the Search and Rescue team initiated a controlled cutting and breaking operation to access the suspected void space. The canine’s detection proved accurate, as rescuers were able to visually confirm the victim’s position within the debris. The search phase lasted approximately two hours, followed by a four-hour technical recovery operation. At approximately 0721 hours, the victim was successfully extricated and subsequently pronounced deceased at the scene.

Shah Alam Flood Rescue & Evacuation
On 18–19 December 2021, Shah Alam Seksyen 25 (Taman Sri Muda) was struck by a devastating flood. What began as a seemingly routine situation rapidly escalated as floodwaters surged to one-storey heights, leaving entire neighbourhoods submerged and homes destroyed. Lt. (CD) Wei Xian, leading his elite team, was the first to reach the disaster zone. The team conducted relentless 25-hour non-stop rescue and evacuation operations, saving stranded residents during one of the most severe floods in the area’s history.

COVID
The COVID-19 pandemic placed unprecedented strain on Malaysia’s healthcare system, with hospitals overwhelmed by rising fatalities. Mortuary facilities, in particular, were stretched beyond capacity, prompting reinforcement from agencies including the Malaysia Civil Defence Force and the Malaysian Armed Forces. Lt. (CD) Wei Xian and his team were deployed to support mortuary operations across multiple hospitals, including Hospital Tengku Ampuan Rahimah, where fatality numbers were exceptionally high. At the peak of the crisis, he managed 30–40 deceased individuals daily—overseeing identification, sanitization, coffin placement, and dignified handover to families, all under sustained infection risk. Concurrently, he supervised several quarantine centers across the Klang Valley and co-developed national SOPs for quarantine management with his superior, later implemented nationwide. The team also operated in EMCO-designated high-risk zones, at times with limited PPE. Throughout the pandemic response, Lt. (CD) Wei Xian led with discipline and resolve—ensuring operational order, protecting his personnel, and serving the community with professionalism and dignity.

Basic Life Support
This course equips participants with the essential skills required to recognize life-threatening emergencies and provide immediate intervention before advanced medical help arrives. Built on internationally recognized resuscitation standards and real-world operational experience, the program emphasizes high-quality CPR, AED usage, and effective team-based response in cardiac and respiratory emergencies.

Basic Pediatric Life Support
Focused on infants and children, this course addresses the unique physiological considerations of pediatric emergencies. Participants will learn structured assessment, pediatric CPR techniques, and choking management tailored specifically for younger victims.

Basic First Aid
This hands-on course provides foundational skills to manage common injuries and medical emergencies prior to professional medical arrival. Emphasis is placed on practical assessment, bleeding control, wound care, fracture stabilization, and emergency scene safety.
Advanced First Aid
Designed for individuals requiring higher-level emergency competency, this course expands on basic first aid principles and introduces advanced trauma management techniques. Participants will train in structured casualty management, airway adjunct usage, spinal precautions, and prolonged care scenarios.

Casualty Make Up Simulation
This specialized course trains participants in realistic casualty simulation techniques used in emergency drills and large-scale exercises. Through professional moulage application, trainees learn to recreate realistic injuries that enhance scenario-based training authenticity.
Swift Water Rescue
This high-risk operational course prepares participants to operate safely and effectively in fast-moving water environments. Drawing from real flood and water rescue missions, the training covers hydrology awareness, personal protective systems, rope rescue techniques, and live victim extraction.
International Life Saving & Bronze Medallion
This internationally benchmarked lifesaving course develops water competency, endurance, and rescue proficiency. Participants undergo structured pool and open-water training focused on drowning prevention, rescue techniques, and casualty management in aquatic environments.
